Award Terms and Descriptions “Tokoh Alumni” Award is to recognize the Leadership shown by any Universiti Sains Malaysia Alumni who has make significant and well-recognised contribution to society in general, and to the specific field of his/her endeavour, in particular.
The Award honours outstanding performance and creative effectiveness in applying knowledge and/or innovative organizational skill in the betterment of the community and furthering the goals of knowledge creation and/or dissemination.
The Award will be presented every two years at an appropriate ceremony to be organised by the Universiti Sains Malaysia Alumni Association. Those chosen as recipients must agree to be present to receive the award.
The recipient will be known as “Tokoh Alumni USM” for the said year. Each recipient will receive a commemorative medal/plague/trophy, attendant media coverage, travel, lodging and related expenses to attend the ceremony.
The recipient’s name will be entered into the “Tokoh Alumni USM’ Honour List/Hall of Fame.
No monetary reward will be associated with the honour.
Eligibility Criteria Nominees must have:
a) a demonstrated record of leadership in their field of endeavours, and
b) served as exemplars and role models to others in their career or areas of work.
It would be advantages if nominees could specify the innovative and creative approaches that they employed in relation to the above.
Applications for Nomination must be received by September 30th, 2006 using the Application Outline enclosed in this leaflet.
Guidelines for Nominations USM Alumni meeting the eligibility criteria and the qualifications set forth under the “Tokoh Alumni” Award descriptions may be nominated.
Nominations may be made by colleagues, co-workers or organizations that are qualified to make such nomination because of their familiarity with the nominee’s service and contributions.
Individuals may not nominate more than one person.
Nomination should be made according to the Application Outline contained in this leaflet and should be accompanied by at least a letter of support.
The nominator must answer completely all questions posed in the Application Outline.
The application may be printed by hand or typed on a typewriter or computer. It may be submittee by mail, fax or e-mail in the same format.
Completed applications will be acknowledged by mail, fax or e-mail depending upon how the application was submitted.
Incomplete applications will not be considered.
The language for submitting nomination is in Bahasa Malaysia or English.
Members of The Award Committee may NOT nominate potential recipients and are NOT themselves eligible to be recipients of individual awards.

Each nomination will be carefully reviewed by an Award Committee.
The Award may be shared jointly by more than one recipient, with a maximum of two recipients. Sharing of award is at the discretion of the Award Committee. The Award Committee also may choose not to give an award if the committee decides the nominees do not meet the award criteria.
Directions for Completing an Application
The application must be completed by the nominator, following the format outlined. Responses may be printed by hand or typed on a typewriter or computer. A letters of support should accompany the application. Organisation letters should be co-signed by leaders of the group. Letter writers are encouraged to include relevant quotes, anecdotes and experiences that illustrate the dedication and concern of the nominee.
